Q. What did this cover inspire ???

Ans. This is Donald Schollander, who won 4 golds in the 64 Olympics making him the recipient of the highest number of medals at a single Olympics. As very evident from his pose, the later holders of this record i.e Mark Spitz and Michael Phelps, paid tribute to him by posing in a similar way.

there was a mad magazine tribute to george bush for topping their dumbest person of the year for a record number of times. was that an allusion to this?

Maybe Spitz's shirtless magazine cover just wearing the 7 medals in 72, which was further copied by Phelps in 08.
